Creating Locations

When I visit the Admin -> Locations section, the New Location button does nothing.  Visiting the section gives the following error in my logs:

MySql Version:    5.1.43
Qcodo Version:    0.3.32 (Qcodo Beta 3)

I am able to create and list new Categories and Manufacturers.

Also, as I am running this with PHP 5.3.x, I have applied the required "patch" recommended in the sticky topic.  Aside from that, this is a vanilla install.  Testing out for a non-profit organization.
